REBATE TERMS & CONDITIONS: Submit: (1) A completed rebate form. (2) A clear and legible copy of the dated invoice, sales receipt, or transaction record for qualifying purchase dated between August 1, 2018 through August 31, 2018, with the purchase price circled.
